Job Enrichment Learning Opportunity Submissions
Formstack is the electronic method for submitting JEP learning Opportunity completions.
- It is the fastest way to submit your learning completions.
- It’s easy to add attachments including certificates, conference agendas, and other documents relevant to your learning completion.
- You can easily track all your submitted learning opportunities. Effective immediately you will begin receiving an email copy of your submissions.
- You no longer need to print forms and attachments.
- Your supervisor automatically receives an email requesting their input and approval.
- Your MyCard record is updated with appropriate points upon supervisor approval.
- Formstack submissions will be the ONLY way to submit your JEP learning completions effective July 1, 2020.
Scanned copies or photos of completed paper forms will be accepted until July 1
Any completed paper forms that have not been submitted can be emailed to StaffDevelopment@Miamioh.edu.
- Don’t have a scanner? Take digital pictures and email the forms and documents.
- Use the free phone app ScanGenius to scan and email forms and documents.
Reminder!
All learning opportunities must be submitted within 6 months of completion.
