Death of Immediate Family Member

When a family member passes away, it can be a difficult and stressful time for you and your family. The University offers several resources to assist you and your family in coping during this trying time.

Time Off for Bereavement

Contact your supervisor to request any time off needed.

  • For immediate family members, you may use up to five days of available sick leave.
  • For others, you must use available vacation, personal, or comp time with the permission of your supervisor.

Dependents Covered Under Miami Benefits

  • If covered by voluntary life that you elected, contact the Benefits office at 513-529-3120 to begin the claim process.
  • If covered under health, dental, or vision insurance plans, complete the Insurance Coverage Change form to end coverage.
  • If listed as a beneficiary under your life or accidental death insurance, complete the Beneficiary Election form (PDF) to update.

Updating Tax Information

You may want to review your federal and state tax withholdings to make any necessary adjustments. This can be done in BannerWeb, or by contacting payroll services for assistance at 513-529-6223.

Do I need to submit a copy of the death certificate?

Not to Miami. If you had life insurance coverage for the dependent, once the Benefits office submits the claim, you will be contacted by Liberty Mutual to submit an original death certificate to them.

Do I need to contact the health, dental and vision providers of the death?

No, the Benefits office will do that.

What if my dependent was listed as beneficiary for my retirement?

Contact OPERS at 800-222-7377 or STRS at 888-227-7877 to change your beneficiary with them.
