Steps to Take Within 31 days if Adding the Baby to Your Insurance

  1. Complete and submit the Insurance Coverage Change form to add the baby to continue coverage.
  2. Submit a photocopy of certified birth certificate as soon as you get it, or hospital verification of birth, or immigration papers that identify parent-child relationship.
  3. Call the Benefits office at 513-529-3926 with social security number. Never email social security numbers.
  4. If you are currently enrolled in voluntary life insurance for yourself, you may now elect voluntary coverage for your child.

Updating Tax Information

You may want to review your federal and state tax withholdings and make any necessary adjustments in BannerWeb, or by contacting payroll services at 513-529-6223 for assistance.

If you are already enrolled in family dental and/or vision coverage, then we recommend that you add the baby now, to eliminate the possibility of the child not being covered when services are needed in the future.
