Visiting the Clinic

Patient Paperwork

Once your appointment has been scheduled, the Speech and Hearing Clinic will send you an email from spaclinic@MiamiOH.edu with several forms to complete or mail a packet of forms to your home. Please complete the forms, sign where required, and bring with you to your appointment.

Please note that if the paperwork is mailed to your home, depending on your appointment date, you may not receive it in time to complete it before your appointment. If that is the case, please arrive 15 minutes before your scheduled appointment in order to complete the paperwork in the clinic.

You should receive the email within 48 hours of scheduling your appointment, so make sure that you are checking your spam/junk mail! If you do not receive the email within the two days, please call the the clinic at 513-529-2500.

Patient Notice – COVID-19

The health and safety of our patients and our staff is a primary concern for our Clinic during this time of COVID-19. You can feel confident when you make an appointment with our professionals knowing that we have revisited every step taken from the check-in process, throughout the exam and therapy to check-out time. We have evaluated current regulations and will implement them throughout your visit. The staff will be wearing protective equipment. The University requires you to wear your face mask as you enter the building, but we will have a face mask in the office available to you if needed.  Employees continue to sanitize their hands between patients and there will be hand sanitizer available for everyone. We have also modified the clinic schedule to allow for rigorous disinfecting of the patient area between appointments so the number of patients seen in a day may be slightly less than in the past.
