An appointment is the best way to access our services. Appointments can be made online via MyChart, by phone at 513-529-3000 or in person. For specialty departments – gynecology, overseas immunizations and ortho/physical therapy – or a physical, appointments must be made on the phone or in person. We will accommodate walk-ins as best we can.

Please bring a legal ID (e.g. driver's license or passport) and insurance card.

If you wish to pay your co-pay on the day of service, you may use a flex-spending card. We will file a claim with your insurance company and, once we receive payment/an EOB (explanation of benefits), anything deemed by your insurance company to be “patient responsibility” goes on the student’s bursar bill. This transaction will be dated on the day it is posted, not by the date of service.

If you are under 18 years old, we need your parent’s written permission to treat you.

If you are 18 or older, we need your written permission before any information is released to anyone. Permission can be granted by signing an Involvement of Care/HIPAA disclosure form at the Student Health Service.

Late / Missed Appointments

You must arrive, per instructions, before your appointment time. If you are more than 5 minutes past your time, you have missed your appointment. We charge $20 for a missed appointment.
