1.  Removal of the Medical Hold. When students wish to re-enroll at Miami University, they need to submit to the Dean of Students a completed Provider Return from Medical Leave of Absence Form (MiamiOH.edu/medical-leave). Once the completed form has been received, a decision regarding re-enrollment will be made and the student will be notified. Students will be prevented from re-enrolling (and registering for classes) if there are any holds on their account, including the medical hold.
