W-2s and the new 1095-C forms are now available

W-2s and 1095-C forms are available online via BannerWeb. 

Two-Factor Authentication is required to access both of these forms. Please refer to the IT services website for information about the Two-Factor Authentication.

Payroll services, Room 2, and human resources, Room 15, in Roudebush Hall have kiosks that are set up for employees to be able to access and print their W-2 and 1095-C forms from BannerWeb.

The 1095-C form is new for the 2015 tax year and provides information that will be used to complete the 2015 federal tax return. Full-time employees receive this form that contains detailed information about their health coverage in calendar year 2015. The Benefits & Wellness website has additional information about the form.

For all USPS mail requests of W-2s and 1095-C forms please contact the payroll services office, and include your name, the address to which your documents should be mailed, Banner ID, and the last four digits of your SSN. 

We cannot email W-2s or 1095-C forms to anyone, due to security risks.  The payroll services office will also print W-2s and 1095-C forms for individuals who come to our office with appropriate ID.
